NURTURED MOMS, LLC Terms and conditions

Lactation Counselor Services

  1. Services Offered: Consultant, Kristen N. Greco, RN, BSN PCD (CBI), CLC ("Counselor") offering lactation counseling services through Nurtured Moms, LLC ("Company"), agrees to provide the following services to Client:

    • Breastfeeding education and counseling

    • Assessment of latch and breastfeeding positions

    • Guidance on milk expression and breastfeeding equipment

    • Providing information on proper infant feeding techniques

  2. Fees and Payment for Lactation Counseling Services:

    • Fees for lactation consultant services are located in the description of each individual service.

    • Payment for lactation consultant services is due at the time of booking. All payments must be made to Nurtured Moms, LLC.

 

Postpartum Doula Services

  1. Services Offered: Counselor, offering postpartum doula services through Company, agrees to provide the following services to Client:

    • Emotional and physical support for the new mother

    • Assistance with newborn care, including feeding, diapering, and bathing

    • Light household tasks related to the newborn's care

    • Providing resources and referrals for additional support as needed

  2. Fees and Payment for Postpartum Doula Services:

    • A non-refundable $30 registration fee is required upon booking postpartum doula services. This fee reserves the Client's spot on the Counselor's schedule.

    • Fees for postpartum doula services are located in the description of each individual package.

    • Payment for postpartum doula services is due at the time of booking.  All payments must be made to Nurtured Moms, LLC.

 

Cancellation and Rescheduling Policy

  1. Lactation Counseling Services:

    • Any cancellation of lactation counseling services must be done at least 24 hours prior to the scheduled appointment.

    • Cancellations made less than 24 hours before the appointment will result in a $25 cancellation fee.

    • Services canceled less than 24 hours before the appointment will not be refunded.

    • Rescheduling of lactation counseling appointments must be done at least 48 hours prior to the scheduled appointment.

  2. Postpartum Doula Services:

    • Cancellations made for postpartum doula services must be made at least 2 weeks in advance of the scheduled services.

    • If cancellation is made within 2 weeks of the scheduled services, a 50% refund will be issued.

    • Cancellations made less than 48 hours before the appointment will not be refunded.

    • Rescheduling of postpartum doula appointments must be done at least 48 hours prior to the scheduled appointment.

    • If trying to reschedule within 48 hours of service, a $50 rescheduling fee will be charged.

Limitation of Liability

Client acknowledges that Counselor and/or Company cannot guarantee specific outcomes or results from the lactation counseling services provided. Counselor and Company shall not be liable for any damages, whether direct, indirect, incidental, or consequential, arising out of or in connection with the services provided under this Agreement, except for damages caused by Counselor's gross negligence or willful misconduct.  Client understands and acknowledges that Counselor is a representative of Company, that Company is a Pennsylvania limited liability company, that this Agreement is being entered into between Client and Company, and consequently, Counselor will have NO personal liability for services offered in accordance with this Agreement.

 

Indemnification

Client agrees to indemnify and hold harmless Counselor and Company from any claims, damages, liabilities, costs, and expenses, including reasonable attorney's fees, arising out of or in connection with Client's breach of this Agreement or any negligent or wrongful acts or omissions of Client.

 

Confidentiality

Both parties agree to maintain the confidentiality of all protected health information (PHI) as defined by HIPAA and other applicable laws and regulations. Counselor agrees to implement appropriate safeguards to prevent unauthorized access to or disclosure of PHI.

 

Governing Law

This Agreement shall be governed by and construed in accordance with the laws of Pennsylvania.  Any claims or disputes regarding this Agreement MUST be brought in Beaver County, Pennsylvania, and both Client and Company acknowledge and accept jurisdiction and venue shall be in Beaver County, Pennsylvania.
